STEP 1: FILL OUT THE FORM



1. In this section you have to complete the form, and attach the copy of your prescription from your doctor. This will help our staff to assist you with the right procedure.

(Sa seksyong ito kailangan mong kumpletuhin ang form, at ilakip ang kopya ng iyong reseta mula sa iyong doktor. Makakatulong ito sa aming mga staff na tulungan ka sa tamang pamamaraan.)


2. You can also check the estimated price range of your prescribed procedure.

(Maaari mo ring makita ang tinantyang hanay ng presyo ng procedure.)

STEP 2: Settle your payment for confirmation



1. Once our personnel/staff verified your procedure and its price via e-mail.

(Kapag na-verify na ng aming mga staff ang iyong procedure at ang presyo nito sa pamamagitan ng e-mail.)


2. You can now settle your payment online to confirm your booking.

(Maaari mo na bayaran `online upang kumpirmahin ang iyong booking.)


3. Kindly provide us the screenshot of your payment by sending it back to our e-mail.

(Mangyaring ibigay sa amin ang screenshot ng iyong pagbabayad sa pamamagitan ng pagpapadala nito pabalik sa aming e-mail.)


4. You will receive instructions on everything you need to prepare before the procedure.​

(Makakatanggap ka ng mga tagubilin sa lahat ng kailangan mong ihanda bago ang procedure.​)


PAYMENT BUTTON

GUIDELINES:

1. You can book your appointment for at least a day (1) or two (2) days.

(Maaari kang mag-book ng iyong appointment nang hindi bababa sa isang araw (1) o dalawang (2) araw.)


2. Cancellation & Change of schedule are allowed considering it will be done 12-hours prior to your schedule.

(Ang pagkansela at pagbabago ng iskedyul ay pinapayagan kung isasaalang-alang na ito ay gagawin 12-oras bago ang iyong iskedyul.)

  • Kindly notify us ahead of time by simply "clicking" cancel/change of appointment on your booking form.
  • (Mangyaring abisuhan kami nang maaga sa pamamagitan lamang ng "pag-click" sa pagkansela/pagbabago ng appointment sa iyong booking form.)


STEP 3: ARRIVE 15-30 MINUTES EARLIER.



Non-appearance from your scheduled date without notifying us will subject to non-refundable privileges.

(Mataas ang tsansa na hindi namin maibabalik ang iyong bayad kung hindi ka magpapakita mula sa iyong naka-iskedyul na petsa nang hindi nag-aabiso sa amin.)


REMINDERS:

1. No show, no information, NO REFUND.

2. CANCELLATION will subject a fee of 5%.
